2 forms 50% success

Hi,

I have two sites running the same form with the same host: galaxygig

With the settings the same on both forms only the older form will send mail.

Form not sending: http://pr.lhandal.com/contact-me/

I have gone through the settings to the point of frustration

I need help please, thanks in advance.
