User registration information

I use your plugin on my site at http://www.camtechcustom.com/dealer-application/ for dealers to register for a discount. The information is supposed to be stored in their user account on the backend, but when the new user is created each field from the form has the following displayed: “Array,Array,Array,Array,Array,Array,Array,Array,Array,Array,Array,Array,Array,Array”

Whats going on here, how can I fix it?

I also do not receive a notification when someone has filled out this form. Part two of the problem.
