Testers Name and Email before doing Test

    Hi there.

    Before taking a test, I’d like to receive the user (testers) email address and name. However, the website / plug-in needs to remember and leave this option when the user already filled in their information once. These email addresses / names I’d like to export to .CSV file once a month.

    Hope someone can help me out!
