Sign-up form not working…

Hi, i’ve used Chimpmate on another website and it worked fine. This time around there is an error and I can’t subscribe to the newsletter once the widget is installed on the website. I’ve added the api key etc… Might be an error on behalf i’m sure. Just wondering if I could get help on the issue?
