form that captures post title

I am looking for a plugin form that will process a posts id/title.

Scenario: Add form to sidebar. When a user fills in the form it will capture all form data and also the post title.

Why: When a user is on a specific page and wants more details about the specific page I want to know what page they have enquired about.
