Pretty good little form plugin

I needed to make an art show submission form and was able to figure it out despite no template. A couple of glitches were settled quickly when I posted questions in the forum. The issues were mainly about how to change some style features. Seems like changing the style (color, background, etc.) on the themes just didn’t take. My only hesitation for the 5th star was that while the developers fixed what I wanted, I still do not know how they did it.
