Cannot attach DOCX documents

Hello,
For some reason, the plugin started acting up when a registrant attaches a DOX document. Attachment format allowed were; doc, docx and PDF. I had to restrict to PDF or user would get a to an error page with a URL ending in /#FSContact1
with this message in the body:
The executive-temps.com page isn’t working
executive-temps.com didn’t send any data.
ERR_EMPTY_RESPONSE
Then the site is unresponsibe/unreachable for about 1 minute.My site: http://www.executive-temps.com
NOTEs: 1/ although I restricted to PDF attachment, the pluging still accept DOCX and lead to the error message,
2/ there are two attachments allowed. the message regarding acceptable file types and size only shows once (and should show twice I guess).Thanks for your help!

Hi, just one question is the file type upper case or lower case? Can you carry out a test, disable all your other plugins except this one then carry out another test in regards to attaching a file. Have you also tested different browsers?

File type is in lower case.
I ran a test with all other plugins deactivated and got the same error.
I tried with Chrome, IE 11 and Edge.PDF is causing no problem at all.

PS: the issue started recently although I cannot say when. I was made aware of the issue by users. I figured out myself that the docx documents were the problem. i have not tested a doc file. Should I?
Ok. i just tried a doc document and got the message that that type of document is not allowed (which is correct as I only allowed pdf).
Nowm why do docx go throu (allowed or not allowed) and lead to that error I mentioned before…?I am wondering if this is a server related issue? Do you know if your host made any changes to your server?

What other changes did you make in your site before this issue started?
The site is hosted by GoDaddy. I have no clue if they made any recent change to the server.

8/8/2016: update to Fast Secure CF 4.0.43, update to Jetpack 4.1.1, update to Yoast SEO 3.4.28/15/2016 (yesterday… trying to fix the problem, but the problem existed before these upadtes): update of theme, update to Revolution Slider 5.2.6, update to WP-Optimize 1.9.1
8/16/2016 (today): update to WordPress 4.6, update to BackWPup 3.3.4.
I just carried out a test in local testing platform and the form works correctly for me. I only allowed pdf files and when I tried to attach a txt or png or doc it gave me an error message which is correct.
My testing platforms runs the following.
WordPress 4.5.3
PHP Version : 5.6.23
MySQL Version : 5.5.5and a great number of plugins. You might want to check with GoDaddy support and ask them to check this issue for you.

Have you tested with docx extension (the most current Word extension – which seems to be the problem)
I just carried out at test only allowing docx files to be attached and it work for me. The browser I used is Windows Edge.
