The plugin was working perfectly, but recently it has been broken

The plugin wasn’t sending through email but it wasn’t working fine before. I found out that if I have enabled the sender information in the footer, then I don’t receive any email.
But if I disable the sender information option, then it is working fine again.

Any thoughts?

Thank you, can you try the following. Disable all your other plugins except this one and then carry out a test? If it works then you know you have a plugin conflict. If it doesn’t work then I suspect it has to do with your theme version or your WordPress version.

Tried it all, now that I have updated to wp 4.5.3
The issue remained the same.

Some mail systems block the message as spam with sender information turned on, if that is the case, you must have this setting turned off.
