How to use mailchimp ?

Hello i d’like to use your pluging to make an email popup suscription.

on the second step of email template: Email form configuration

I don’t know how to integrate mailchimp,i always have an error:
“Error! It seems that there’s no form present, please edit your code and try again.”
i tryed to put shortcode, html, id ……..

Could you help me plz !
