Which item are you trying to unhide, specifically? What are the default permissions of that item (e.g. what’s the required capability)?
One thing you could try is clicking the little pencil icon next to the Permissions / Required capability field. This will reset permissions to default for that item.
The icon shows again on the item when in the left column, when I view it in the right column to edit the icon is not there.
What do you mean by left column / right column in this context? Are you referring to the two columns in the menu editor (in that case, how can the same item be in both?), or the actual admin menu vs the menu list in the editor?

Yes, in the menu editor.
When I edit the menu item (a plugin in this case) it appears to the right (ie submenu) in the menu editor so I can access the edit options. Resetting capabilities did not solve the issue. It remains hidden in the admin menu.
Are you able to change menu settings that are not related to visibility or permissions, like the menu title? Can you hide and unhide other menu items? This would help determine if it’s a general problem with saving settings, or something permissions-related, or an issue specific to that particular menu item.
Also, could you tell me which plugin and menu item you’re trying to enable/unhide? If the plugin is publicly available, I’ll install it on a test site and see if I can reproduce the problem, and check for plugin conflicts.
