    @enibbe,

    Please add the following to your Form Builder CSS or if you have free version to contact_form_maker_frontend.css file of the plugin:

    .forlabs {
    float: left !important;
    }
    .wdform-ch-rad-label {
    float: none !important;
    }

    This will resolve your issue. Thanks.
