Having this same issue. No form fields showing up on the application page, only the Attach Resume choose file option.

Yes, I have created 6 form fields in Settings / Application Form Fields
Settings form fields only populate to job detail page when you create a new job.
Please first add fields in Settings & then create new jobs.

After i completed the recommended steps I am getting an error404 message when I click on the job posting.
Please advise!
404 Page Error:
It’s a permalink issue. Please reset.
For reset, go to:
Setting > Permalinks
[NOTE: Be remembered your selected option]
Select Default option, hit save.
Select Your previously selected option, hit save again.

How will this affect my current work on my site?
Never.
It won’t disturb your current work. You can simply go on the permalinks page, without changing any option just hit SAVE.
It will work fine. It’s common in many plugins where new slugs are being generated through plugin.
