I’m not sure what you’re referring to. You add custom fields to WP to Twitter templates by using the pattern custom_field_name – but WP to Twitter doesn’t create those custom fields for you or manage them; that’s just a way for you to leverage custom fields provided by other plug-ins.
I don’t know what image_link is or what it refers to…

Well, if you know the custom field name, you can insert the link to the image using custom_field. But if you want to upload the image to Twitter, then that’s not an option that’s available in the free plug-in; you’d have to purchase WP Tweets PRO.
