Contact Forum Builder Plugin Vanished

After upgrading to WP 4.5 yesterday it seems my Contact Form Builder plugin has vanished. It doesn’t show up in my active or inactive plugins lists. I noticed this by checking on my “contact” page.

My first thought was to try to re-install the plugin. I get this message when I attempt that.
