Admin Menu/Page is blank

I was trying to duplicate a service. I was using the 3rd party migration tool to see if I could accomplish this. Now anytime I try to go to the admin menu admin.php?page=forms-3rdparty-integration-config it’s just a blank screen. I even deleted and re-installed the plugin to no avial.. help!
