Forms Not Working

When someone signs up through the comments, everything is fine. They receive their confirmation notice and eventually the newsletter. However, I have two forms that are not working, neither with Genesis eNews Extended (top) or with MailPoet Subscription Form (bottom). With eNews, e-mails are being added to subscriber list but remain unconfirmed because, of course, confirmation notices aren’t going out.

I’ve looked and can’t find a solution. MailPoet has sent me here for one. Please help.
