    I have seen this issue before which was caused by a theme conflict. One way to test this would be to temporarily enable a default WordPress theme like Twenty Fifteen to see if it resolves the issue.

    It seems to me that it has to do with a javascript not always running properly. With the same theme (Avada) 90% of the time the form works (and a javascript transforms divs like <div id="wc-paypal-braintree-expiration-date"></div> into input fields), and 10% of the time it does not. This behavior happens in at least Firefox and Chrome.
