Fields should be only shown in gallery context

Hi,

Neat plugin! A suggestion: gallery fields should only be shown while editing/creating a gallery. Right now, these fields are also displayed when attaching a single image, as well when editing an image from within the library.
Seeing these fields when not necessary create confusion.
