Forgot Username form

The Forgot Username form is currently part of the [wp-members page=”user-profile”] shortcode, which doesn’t appear to be set up on your site. You have a “user-profile” page, but it appears that you are using the [wpmem_form password] shortcode on that page. You need the user profile shortcode referenced above for it to work.
