The image can be replaced by navigating to settings Options and uploading a new image in the option for “Custom ‘Details’ Icon”. The new image should appear right away.
I tried that and now the icon has disappeared altogether. Is there a particular size it needs to be?

32*32px or smaller would be the size you’d want your icon to be. Let us know if it reappears 😉

No it doesn’t. Resized, reuploaded and it doesn’t show up.
The boxes also seem to have broken – you can see that there’s gaps in the borders. how do I fix it?

It is displaying a 404 error. Would it be possible that you have implemented any custom permissions?
Does your .htaccess file have the following:
<Files *.php>
deny from all
</Files>
Soemtimes, there is insertion of a .htaccess file into wp-content/uploads/ and which doesn’t handle calls properly afterwards. People have been able to fix it by deleting the htaccess file
