app_confirmation parameters not displaying

Hi there, I am trying the free version of this plugin to make sure it can be configured for our client before we buy it. However, we’ve run into an issue where the form fields for the app_confirmation do not appear. These are the shortcodes we have on the page:

[app_services autorefresh=”1″]
[app_monthly_schedule]
[app_confirmation name=”Your name:” email=”Your email:” phone=”Your phone:”]
