Contact forms lost

HELP!

My Hosting Provider required me to migrate over to a newer infrastructure. In doing so, I had to update the path info on all my links, pictures, etc.

I finally got the site back up and running, however, the Contact Forms that were previously created are lost. I suspect a change in path is the reason for this also.

Is there any way to find these forms and add them to my updated instance/path?
