1: Site wide, but limited to requests that are handled by WordPress. So for instance images that are served by the webserver directly will not be blocked. But posts, pages etc will.
2: Use for instance a VPN service that has representation in different countries and also supplied the correct country names for their ip addresses.
3: You can allow search engines in the configuration
4: It will be easier for the webserver to handle DDOS attacks as it will limit the output that it has to send to blocked countries. But it is not designed in any way specifically to mitigate DDOS attacks.

Hi,
I don’t use a proxy or a VPN myself. I I use one of those freeproxies I see my ban message. So I cannot help you there.
Personally I have a couple of VPS servers in different countries
