What version of WordPress are you using as well as your php version, and plugin version?
This is not a know bug, so it may be an isolated incident that we need to try to track down.

Using PHP 5.2 on GoDaddy, Latest version of WP and the plugin

I need to try to troubleshoot the problem and recreate it, so we can possibly tell you what to do to resolve it.

Thanks for the screencast.
One thing, if you can check for me, is when the plugin is active, do you have any errors in your JavaScript console? This is located under the error console.
I have yet to recreate the issue, so I need more information on what might be causing it. It is most likely a JavaScript conflict of some kind.

I figured it out. On my side it wasn’t even related to WP. ALl I had to do was turn off my adblocker in the browser π => http://screencast.com/t/u80nmFMW4aA

I also had this issue with the Amazon Products plugin. I disabled AdBlocker as per Ovidiu’s recommendation and the Visual Editor options reappeared.

I defused all of my popup blockers and then lit them off one at a time, Adblock V2.42.1 & AdBlocker Plus v1.9.3 are the culprits.
Better Pop Up Blocker v2.1.6 played nicely with WP
Great – glad we were all able to figure this one out and let others know the culprit.

@don: you got me onto the right track, checking the js console I noticed several blocked css files belonging to your plugin listed as: blocked by client that’s how I found adblock as the culprit.
