    Doesn’t sound likea W3TC issue but an issue with you either:

    1. Using a known hackable plugin (they are getting a backdoor through it)

    2. You havent updated your wordpress in a while and u did so after they got access (meaning their is a fake php file or other access into your site)

    I recommend you deactivate ALL plugins and put on something like Ultimate Security plugin to analyze your site including comparing what files have been compromised and go from there. Or better yet, delete the entire site and reinstall WordPress from scratch and update ALL plugins and reactivate on the essential ones.
