Form builder page not styling correctly

In the Mail Poet admin under settings the Forms tab/page is not laid out correctly. The sidebar is at the bottom of the page and I cannot change the name of the form or make any modifications. I have deactivated all plugins and tried multiple themes.

Any suggestions?
