Form hides after load

    Hello,

    I am having an issue on webpage.

    My contact form hides after page fully loads or after mouse pointer enters on video player which I have on the same main page.

    Where could be the problem?

    Other important information:
    Using WP 3.9.8 (I am not updating it because I am afraid of problems which I had last time after update to WP4.0 :D)

    I had already updated FORM plugin, but ir does not help.
