bluemi332: Depending on how you define high, my personal point of view would be to agree with you, wp-useronline is not recommended for high traffic sites.
High traffic is something like 100,000 daily pageviews.

100k daily pageviews is definitely a lot and wp-useronline is not very efficient I must admit.
I would add one more thing, even with efficiency problems, the Plugin is quite informative.
Wish it could take less resources.

If your site is on high traffic, I would not even recommend a useronline plugin as no matter how efficient the plugin is, it would still require at least 2-3 queries per page load just for the plugin itself.

If you want to keep track of user activites, you can use following web services. These services keeps track of your visitors, their activites on your site and alot. In addition, you can also display “Total users online”.

2. Site Meter
3. StatCounter
These services’ll not display user activites in real-time. As anything to show realtime’ll definitely take-up lots of your resources.
