2.3 upgrade db error upon page edit

    I just upgraded to 2.3 and whenever I edit a page and click Save I get this error:

    WordPress database error: [Duplicate entry ’18-0′ for key 1]
    INSERT INTO rdc_term_relationships (object_id, term_taxonomy_id) VALUES (’18’, ”)

    Warning: Cannot modify header information – headers already sent by (output started at /home/morglawred/www/www/wp-includes/wp-db.php:160) in /home/morglawred/www/www/wp-includes/pluggable.php on line 391

    I used WP-dbmanager to repair the db but no luck. I tried to restore from the backup I made just before upgrading, but the restore failed.

    This only happens on page edits and not posts. It also only happens the second time I edit and save – the first time it works just fine.

    Any ideas?

    I’ve tried deleting all plugins from the server, and switching to upgraded themes to see if that was the problem. No luck.
