contact form not centered

I have followed all instructions in the FAQs as well as on the mrbam website, but I still cannot get it centered.

I gave the form a blue background and everything is all the way to the left, it looks like this:

I want it to look like this:

I made these in paint, but you get the point. I want the form to look centered and nice. How can I do that?

Another question that I had, which I wasn’t sure if I have to make a new thread for, is whether or not the contact form is compatible with revolution slider. I have a theme that uses revolution slider as a homepage banner, and I want the contact form to be on the top of the banner. will FSCF work for this purpose? any known compatibility issues?
