multiple errors and white screen of death

Hello all,

I have been running a website for a while now. For some odd reason my host deactivated my site and I had to get it reactivated. They had also changed my database nfo, which i believe I have correct. I now get the white screen of death on everything, including the admin page. They did email me the errors and i can post that here. Please help.
