Having just migrated from the old to the new version of the plugin all the files permalinks give 404 errors. I tried uploading a new file with the same result.
The form of the permalink is http://carnethy.com/download/some-filename/

The Download Manager file folder itself is protected by .htaccess to deny all access so I presume the address triggers some php action to retrieve the file. The folder contains both password protected and non password protected files. I don’t know if this is relevant or not.

    Downloading works ok though.

So I could live with that if it were not for the plugin WPDM – Button Templates which give a very much nicer styled download link to a file than the main plugin. The title is a link which is more prominent than the word ‘download’ so users will be tempted to click on it. It however goes to a 404 page.
