I’ve checked your site and I’m not being able to reach the confirmation screen due to some errors that are preventing plugin to work properly, all I’m getting there is spinning icon once I click on the time so we’ll most likely have to deal with that one first.
Can you please try testing for possible plugin conflicts, try using default WordPress theme like Twenty Fifteen and deactivating all other plugins as displayed in the following flowchart to know which theme/plugin is conflicting if any.

As for the issue you’re having with the misaligned field in the form I’m sure we can add some custom CSS to fix that.

Hey again Monaco,
A quick follow up regarding the error you’re having on appointment page. After doing more research on that error it appears that the error you’re having there that is preventing ajax from working properly is related to your SSL certificate.
