What other plugins do you use?
I have been having trouble just this last week. So this newest site I only loaded contact form 7 to make sure this I eliminated as much stuff as possible. So it only has the one plugin.
I just had a new client come to be with this problem today. The visual editor was just fine until recently, now it is missing as well as all the formatting tools.
I turned off several plugins, didn’t get the visual editor back until I deactivated Contact Form 7. Now all plugins are activated, except for CF7, and all is well. I turn CF7 back on and the problem returns.
They are using WP 4.2.2, their theme appears to be official 2.0, the plugins are:

I hadn’t heard anything recently about this issue, but just looked at your Plugin site and saw the last edit date for Contact Form 7 is 6-22-15. I checked my client’s site and the Visual editor is working, so they must have upgraded without telling me.
So… it’s fixed! Thank you!
Thank you Miyoshi. the plugin works without any issues again. Sorry I was out of town but it was a welcome surprise when I returned to work.
