display thumbnails w/ post links- how?

How do I display a list of posts with just a small thumbnail, the title and the date? Right now, I’ve gotten as far as the date and title, but don’t see where to add a thumbnail. It’s a small thumbnail, not a big image I’m after. I have screenshots of what I’m after if you need a reference.

Also, I’d like to add a subscriber form (either from widget or shortcode) in a menu bar as well as a search.
