Form 7 Worked Before Modifications

My Form 7 worked great right out of the box… or so I thought. The emails are going through, but without the “Name” or “Email” data. So then I thought, well, I have to generate those tags. So I generated tags for both fields, and implemented the short codes… but I must have done it wrong, because now the form won’t even submit–gives nondescript error message at bottom.

Is there a graphic somewhere, I wonder, that shows exactly how to paste in the shortcodes? Adding them to the “mail” fields is really a head-scratcher… how exactly? The instructions assume you know sooo much, and leave out vital info.
