Need easy registration form

Hi. Does anyone know of a plugin that allows people to put together an easy to use sign up sheet? What I want is a signup sheet where the person can put their name, address phone number, email address, etc? I’ve tried to use a few recommended plugins to develop a signup sheet but they either were for signing up for a user name and password or just for the user to send their email. All I want is a simple signup sheet. It’s for a site where people can sign up for healthcare. Thanks.
