Email notification – pull 'To' from form?

I understand that for an email notification from the form you can pull the data from a field on the form for the sender, but is it possible to do the same for the recipient of the email?

In other words, I have some radio buttons setup with emails for the values of each, and am trying to make it so that the user can select one of the emails using the radio buttons, and send an email to that email address. Is that possible with this plugin, or would I have to create a separate form for each email address?
