"Name" field not appearing | WordPress.org

I just updated Gravity Forms, the Directory addon, and WordPress, and now the “Name” field isn’t appearing: http://staging2.vrbp.org/verde-river-basin-your-government-directory/carefree/ .

Any initial thoughts?

I can confirm that the separate fields of Name (First) and Name (Last) work fine, it is the field that merges these into one that does not display any information.
