Only works on 1 category

I am trying to get this to work on a top level category which has a number of sub-categories and sub-sub-categories. I doesn’t work unless I only apply it to the the sub-sub-category.
If I add more than one sub-sub-category it doesn’t work again!
The settings I’m using are:
Category – Equal to – sub-sub-category name1
or
Category – Equal to – sub-sub-category name2I add a product from category name1 to the cart and it shows free shipping. I then add a product from category name2 and free shipping no longer shows but the other shipping methods do. If I delete either of those from the cart it shows free shipping again.
Not sure if I’m doing something obviously wrong….
Incidentally I find the Woocommerce shipping debug mode to be completely ineffective and have to purge the database after every test as per this post http://spencerbean.com/manually-purging-woocommerce-shipping-rate-cache/
