getting 404 page

I just downloaded this… looking for something to use for help and FAQ…
I created 2 FAQ’s; no groups.
I created a page with: [faq posts_per_page=”-1″] and added it to the menu.
When I click on “Preview Changes” or click on the page in the menu, I get 404
using WP 4.1
Any ideas?
