Problems after update

Hello,
My form was working great. After updating plug in the grey text is faded into yellow default background. Then I switched to blue, could read the text, yet the box itself looks unappealing. There is a ‘segmented’ row along the top of the box on the same level as the “x” to click out of the box.
I switched back to default yellow and cannot see text. I did not see anywhere in the settings where I could adjust text. Please advise. You can go to my website to view:
