Warnings when use Captcha & Check boxes

Hello!
I have little problem with Contact Form Manager plugin.
I use 4.1 plugin version and WordPress 4.1.
You can see my form by the link:

If enter these data in the fields, several warnings will appear.

Your Name : rt5t54t54t54t
Checkbox : //don’t tick any
OnlyCaptcha : 45y45y //or any wrong value

Click on “Send” and then you will see warnings:
