Thank you for the reply. because you cropped the image partly so that i tried creating a new form like the picture. The function related to address and postal code is running normally (http://i.imgur.com/vktnMYx.png). Could you provide the site login account via the private message on our forum mentioned above so that i can investigate the cause for you better.

Same problem here… With last version of WP 4.2.1.
Saddly I can’t give you access to my dev server.

After a bit of monkeying, it appear that changing :
name="address[' . $data->id . '][code]"
to
name="address[' . $data->id . '][pcode]"
in wr-contactform/helpers/form.php @ line 906 is fixing the issue...
I don't have time to investigate more. Sorry...

It sort out that the problem is caused by a conflict with the plugin :
Bootstrap Shortcodes for WordPress
Disabling JS & CSS from WR ContactForm setting’s won’t fix the issue.
The only way I found until now is to disable the Bootstrap Shortcodes plugin…
I don’t need the Bootstrap Shortcodes plugin as much as WR ContactForm, so I’ll go without it for now.
If you are going to investigate deeper this issue, I’ll be happy to help!
