What changed — the plugin?
Have you installed or updated other plugins (or themes) recently?
Can you post a screenshot of the form settings? Something is very “off”…
Sorry for the vagueness. It was working for a couple of weeks – then it stopped working the day I posted the comment. I believe the theme may have been updated. I am looking for a previous version to see if I can go back and test an earlier version of the theme.

It looks like it may have something to do with field names… I tried creating a new form using the default fields and it submitted no problems… hmmm.
You may be using a reserved query variable like p, page, name, etc.
Can you list the field names you’re using (or screenshot those)?
