date format seems invalid

I created a new contact list and add a date field. Now I am testing and when I include date it showing one message;” date format seems invalid”. I am not able to submit the form, and I need to know which exactly format date is required by the plugin in order to include the sample on form. How can I can fix this issue? Thank very much for any help.
