    Thanks for posting! On your site I definitely do see the form you embedded, but this form appears to be in place using the MailChimp embed form code rather than the MailChimp List Subscribe Form plugin I’m able to support here in this support forum. I’d recommend perhaps trying out the plugin widget on your site since I am able to see that the MailChimp List Subscribe Form plugin appears to be installed in your WordPress. To do this, you can go to Appearance->Widgets in your WordPress admin console and drag the MailChimp Widget widget into one of your Widget Areas.

    For support with the embed form code specifically, I would recommend reaching out directly to MailChimp technical support: http://kb.mailchimp.com/help

    Thank you. Yes, I have the plugin widget in my home page side bar, but I want a separate form specifically in the MuseChaser Movement page, which is why I tried the embedded form code since I can’t place a widget within the page (or can I?).

    You should be able to add the widget to that page as well! One thing to try would be using the shortcode. There are a few options for inserting short code under the “Advanced” section of the Installation page here: https://wordpress.org/plugins/mailchimp/installation/

    Placing [mailchimpsf_form] in the location on your Page where you want the form widget to appear should do the trick!

    Wow that short code worked! Thanks a lot. But now, can I customize the form on that page to look different from my home page widget? I just want to change the title…

    Hi Anokina,

    The shortcode will pull in basically the exact same thing as the widget. The only place to customize the header and subheader for the plugin form is in the plugin settings, and that will apply to all instances of the form. What I would suggest doing if you want different headers would be to leave the header and subheader in the plugin settings blank, and add your own custom headers preceding each form on the site, so they can vary based on your preference.
