Invoice look-up not working

Two questions…

1) To view the invoice look up the user must be logged in, correct?

2) Once the user logs in and tries to look up the invoice, when the users types in his/her invoice # it returns to a 404 page.

This is the short code I’m using on my page for the invoice look up.

This worked before until I updated to WP 4.0 and updated the plugin.
