The Bio Entry Card template does not have a category list. Perhaps on the other site you are using one of the premium templates which support this feature.
What is the link to the site that does have the category list?
re: I have the category description checked in Settings > Display.
The category description is only shown when filtering by category using the categories slug/permalink.

I had to repurchase the Connections-Widgets module as it allowed me to use a Categories Widget on the left side which connected categories to the Bio Entry format entries.
Then I used Say What to rename fields like Work Fax to Lodge Fax (nice little package).

Now, one last question. I imported Notes, but they don’t show up. Is there some piece of this puzzle I’m missing?

None, of the core templates show the notes. Only a few of the premium templates show the notes fields.
You can add this $entry->getNotesBlock(); to the template to show the notes. If you decide to do this I recommend follow the instructions in this QuickTip. So any cahnges you make will be update safe.

Well, since I already own 2 pro templates, I’ve opted to use Slim-plus. The downside to this is that it doesn’t have the Download Address Card like the Market template does, but Slim-plus has a cleaner look than Market, IMHO.

Yes, add image='none' image_fallback='none' to the shortcode so you have something like this depending on if you have any option set or not.
