BPS Blocking form thank-you page

Hello.

We have a form on our website that sends information to our thank-you page so that it can display a custom message depending on what is selected / input in the form. Ever since installing BPS when someone submits the form a 403 Forbidden access error displays.

Here is an example of one of the error log messages:

Any help appreciated in what I need to do to resolve this. When I don’t activate .htaccess in wp-admin, root folder etc.. BPS has all these warnings in the dashboard. When I do, now the form is not working properly.
